Instructions

Permit #: If you know the specific permit number, enter it here. Enter only the number without "-", ".", spaces, etc.
Parcel #: If you know the property's parcel number, enter it here. Enter only the numbers without "-", ".", spaces, etc.
Street Number, Street Name: The address of the property with no directions, apartment numbers, suffixes, etc. So, for example, someone living at 87 Reads Way would enter street # 87 and street name READS.
Do NOT enter suffixes like 'Road', 'Lane', 'Ave.', etc.
Contractor ID:: If the permit was issued to a contractor, the contractor's federal id number can be used.
